L—苹果酸是生物体可以利用的形式，它常配以复合氨基酸注射液（手术后重要的营养药品）中，以提高氨基酸的利用率，这对手术后虚弱和肝功能障碍病人尤其重要。L—苹果酸钾是良好的钾补充药，它能保持人体的水分平衡，治疗水肿、高血压和脂肪积聚等症。L—苹果酸是治疗肝病，尤其是肝功能障碍导致的高血氨症的良好药物。L—苹果酸钠具有食盐的1/3咸味，可作肾脏病人代食盐。
L—苹果酸在食品、医药、日用化工等部门的用途如下：（1）食品工业：用做加工和配置饮料、露酒、果汁，也用于糖果、果酱等的制造，对食品且有抑菌防腐作用。亦可用于酸乳发酵PH调节，葡萄酒酿造中除酒石酸盐等。（2）烟草行业：苹果酸衍生物（如酯类）能改善烟草香味。（3）医药工业：各种片剂、糖浆配以苹果酸可以呈水果味，并有利于在体内吸收、扩散。（4）日用化工工业：是良好的络合剂、酯剂，用于牙膏配方、净牙片配合、合成香料配方等，还可以作为防臭剂和洗涤剂的成分。
比旋度取本品，精密称定，加水溶解并定量稀释制成每1ml中含0.085g的溶液，依法测定（中国药典2010年版二部附录VI E），比旋度为－1.6°～－2.6°。
【鉴别】（1）取本品约0.5g，加水10ml使溶解，用氨水调pH值至中性，加1%对氨基苯磺酸溶液1ml，在沸水浴中加热5分钟，加20%亚硝酸钠溶液5ml，置水浴中加热3分钟，加4%氢氧化钠溶液5ml，溶液应立即呈红色。
（2）本品的红外光吸收图谱应与L-苹果酸对照品的图谱一致（中国药典2010年版二部附录Ⅳ C）
